Home Health Aide (HHA) – Resident Transport

The Phoenix Rehabilitation and Nursing Center is seeking a reliable and compassionate Home Health Aide (HHA) to assist with the safe transport of residents within and outside the facility. This is a vital role that ensures residents arrive safely to appointments, activities, and therapy sessions while providing friendly and supportive care.

This position is primarily focused on resident transportation, making it ideal for candidates who enjoy working directly with seniors and supporting their mobility and well-being.

Home Health Aide (HHA) Responsibilities

Safely transport residents to and from medical appointments, therapy sessions, and facility activities
Provide assistance and support to residents during transfers and mobility challenges
Monitor and ensure resident safety and comfort during transport
Communicate effectively with the care team regarding resident needs and observations
Assist with light resident care and companionship as needed
Follow all facility policies, procedures, and safety guidelines

Home Health Aide (HHA) Qualifications

HHA certification in good standing
Valid driver’s license and clean driving record (if transporting offsite)
Ability to lift and assist residents safely
Strong communication and interpersonal skills
Compassionate, patient, and resident-focused
Prior experience in a skilled nursing or rehabilitation setting preferred but not required

Pay Rate: $18 per hour
