WeCare at South Hills Care Center, located at 200 Village Drive, Canonsburg, PA, 15317 is actively seeking dedicated per diem, LPNs to add to our growing team!

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

  • Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment. 

  • Provide regular patient status updates.

  • Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

  • Complete required documentation in an accurate and timely manner.

  • Supervise, administer and or assist with medications.

  • Provide direct care of residents as necessary.

  • Follow the plan of care for each resident.

  • Ensure the residents care standards are maintained.

  • Report to the Director of Nursing.
