Job Summary:

Meadow Falls of Conneaut is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our dedicated healthcare team. As a vital member of our interdisciplinary team, the LPN will provide high-quality care to our residents, ensuring their physical, emotional, and social needs are met with dignity and respect.

Responsibilities:

Provide direct patient care, including administering medications, wound care, and vital signs
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor and document patient progress, reporting any changes or concerns to the RN or physician
Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
Maintain accurate records and charts, adhering to facility policies and procedures
Demonstrate excellent communication and interpersonal skills, working effectively with patients, families, and colleagues

Requirements:

Must be a graduate of an accredited LPN program and hold a current state LPN license
Minimum of 0 years of experience, with a strong orientation to learning and growth
Ability to lift, transfer, and reposition patients, as well as move equipment and supplies
Strong attention to detail, with excellent observation and documentation skills
Compassionate and empathetic approach to patient care, with a focus on resident-centered care
