What You Must Have

  • Active LPN license in good standing in the state of practice.

  • Experience in a skilled nursing or long-term care setting is preferred but not required—we’ll invest in the right heart and mindset.

This position must integrate company values into daily practice. Essential Functions: Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse. Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN. Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient. Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary. Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs. Helps identify patient problems and potential risks and contributes to the development of interventions. Assists in identification of patient’s learning needs and reinforces self-care and other techniques.
