MDS Coordinator - SNF

Located in The Bronx, NY

Salary: $125K to $165K range (based on experience)

Responsibilities of the MDS Coordinator-Nursing Home:

  • Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned
  • Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines
  • Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
  • Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents.
  • Initiates care plans and supporting activities that will result in best possible outcome for assigned residents.
  • Generates and distributes monthly care plan calendar for the following month.
  • Conducts care plan conferences for assigned residents.

Requirements of the MDS Coordinator-Nursing Home:

  • Must have a current New York RN license
  • Must have long-term care experience
  • Must have current experience as an MDS Coordinator
  • Must know MDS 3.0
  • Have a strong understanding of the MDS process 
  • Have strong decision-making and problem-solving skills

MDS Coordinator (RN) Mostly Remote Position

Evergreen Commons is looking to hire a full-time, Monday-Friday day position, dynamic MDS nurse to work at our skilled nursing facility. This role includes flexible hours, mostly remote with 1-2 days a week on site. Your duties will include facilitating the MDS process and assessments of residents as necessary.
