A skilled nursing facility is seeking an Admissions RN to join our team.

Position Overview

The Admissions RN is responsible for coordinating and overseeing the clinical admission process, ensuring a smooth transition for incoming residents while maintaining compliance with all regulatory and facility standards.

Schedule

  • 1:00 PM – 9:00 PM

Key Responsibilities

  • Conduct admission assessments for new residents and hospital returns; initiate care plans
  • Review PRIs and coordinate admissions with the interdisciplinary team
  • Introduction of new residents to staff, unit, and facility
  • Ensure all documentation and charting are completed accurately and in a timely manner
  • Audit records to maintain compliance with facility policies and regulatory requirements
  • Participate in discharge planning when needed
  • Evaluate and update resident care plans based on condition changes
  • Complete MDS and related documentation as assigned
  • Maintain strict confidentiality of all resident information
  • Provide clinical guidance and supervision as appropriate
  • Conduct other duties as needed

Qualifications

  • Active, unrestricted New York State RN license
  • Prior experience in admissions within a long-term care setting
  • Strong clinical assessment and independent decision-making skills
  • Leadership and supervisory experience preferred
  • Proficient in Microsoft Office
  • Excellent communication and interpersonal skills
  • Familiarity with Sigma is a plus
