Responsibilities:

Provide personal care services to our residents, including bathing, dressing, and grooming
Administer medications and treatments as directed by the supervising physician
Monitor and report changes in residents' conditions to the supervising nurse
Collaborate with other members of the care team to develop and implement individualized care plans
Maintain accurate and up-to-date resident records
Participate in ongoing training and professional development to stay current with best practices in the field

Shift and Schedule:

12-hour full-time night shifts (including rotating weekends)
Part-time weekend shifts available

Pay and Benefits:

Starting hourly rate: $27.50hr & up
*Up to $3/hour shift differential available

Requirements:

Current and good standing Licensed Practical Nurse (LPN) license in the state of Missouri
Ability to work effectively in a fast-paced environment
Strong communication and interpersonal skills

The Clinical Patient Navigator manages a variety of front office functions and is key to smooth operation of a dynamic outpatient wound care center as well as performs general office duties to assist the staff of the WCC. This individual serves in a dual administrative/clinical role to streamline workflow in providing effective and exceptional patient care.
