Responsibilities

Under the direction of the RN, the LVN will:

·     Assist in identifying the physical and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team reports, and evaluations

·     Participate in planning and implementing care in conjunction with the RN

·     Provide care utilizing infection control measures that protect both staff and patients

·     Assure continuity of quality patient care delivered with appropriate documentation

·     Prepare regular clinical and progress notes and summaries

·     Assist patients with activities of daily living while teaching appropriate self-care techniques

·     Provide and maintain a safe environment for the patient, including an optimum physical and emotional environment

·     Maintain strict client confidentiality at all times

·     Follow emergency procedures in the event of any catastrophic event or incident, including accidents, injuries, or significant changes in a patient’s condition

 

Qualifications

·     Graduate of an accredited school of vocational nursing

·     Proof of current LVN license required

·     Proof of current CPR certification required

·     Ability to work a flexible schedule

·     Minimum 1 year of experience as an LVN in a clinical setting preferred

·     Excellent interpersonal and communication skills

·     Valid driver’s license and reliable transportation
