Position Summary
This position is responsible for the delivery of patient care that promotes safety and well-being of our patients across several programs. This position will be responsible for the patient interaction when they arrive for the appointments, collect vitals, blood or urine samples and prepare for sending off and provide a hand off for the nurse practitioner. The clinic nurse will administer injections for patients as needed both in the clinic setting, but could also be in the patients home or community if they are in the ACT or IRSH programming. This position includes reviewing medical faxes for information and uploading to the electronic health record, communicate with pharmacies regarding medications, refills for patients along with medication reconciliation, prior authorizations, and coordination of services.
Job Responsibilities
· Administer, monitor staff initial and continued TB testing for job duties and ongoing compliance
· Track and monitor all client incident reports, Collaborate with Director of Operations annually for reporting of any trends
· Thoroughly understand testing (lab and procedures) values and relationship to patient status with accountability to take next steps, as appropriate to scope and licensure.
· Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines.
· Maintain compliance with Personnel policies and procedures.
· File compliance regarding nursing/medical areas for all clients in programs listed below.
· Monitor and report on all mental health court committals
Strive Recovery Program
· Weekly medical group for PHP/IOP level of care – document as required in the EHR
· Assist clients with getting signed in for provider visits
· Update provider as needed
· Administer injections or medications to clients per prescriber orders
· Ensure medication refills are completed when needed
· Update social history regarding health problems according to protocols.
· Document accurate information regarding health concerns and/or condition from patient or family member.
· Provide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processes.
· Facilitate and support relationships between providers (e.g. within clinic, referring) and with ancillary services.
· Demonstrates competence in the clinical skills necessary to carry out assigned job duties.
· Complete appropriate forms, assessments or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-9 coding.
· Follow up on medical records
· Follow ups on pharmacy calls, faxes
· Uploading documentation into EHR
· Run reports from PMP (prescription monitoring plan)
· Processes medication requests/changes/refills per established written protocols. This includes having knowledge of medications, including basic dosages, side effects and interactions.
· Assist provider and other clinical staff with obtaining basic vital information as needed.
Assertive Community Treatment
· Work directly with program participants to assess physical health care needs and coordinate with member’s health care professionals on those needs.
· ACT home visits should be for medical / medication needs only unless crisis situation or approved when short staffed.
· Administer injections to clients as needed, or other medications
· Communicate and coordinate with primary care and specialty care providers to review and plan care for members.
· Assist in coordination of medically related service needs and supports.
· Understands self-management tools, usage and need and can develop/organize for the population being served
· Follow all standards of ACT services
· Enter data as required for member outcomes, comfortable with computer use.
· Implementing preventive, as well as remedial, health and wellness plans.
· Work closely with other members of the team and staff from other key agencies.
· Provide education and training to agency staff regarding health issues.
· Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
· Abides by all specific program and Agency procedures, policies, and requirements.
· Complete nursing assessments and medication reconciliation for clients as needed
· Provide medical and psychiatric education to clients and their families
· Conducts medical/health/wellness assessments to identify health care needs and assist in developing health care goals with the treatment team.
· Track required healthcare assessment and screenings
· Having knowledge of physical health aspects of client care, including weight, nutrition, medication education, assessing and education of side effects, risks and benefits of medication
IRSH
· Assist with getting clients into system for provider visits (If it is with our prescriber)
· Administer injections/ medications to IRSH clients as needed
· Provide supervision/training and monitoring to staff for medication management administration.
· Review client MARS monthly
· Coach/Train staff on medication errors
· Monitor all medical/medication related incident reports for IRSH clients

Qualifications
· RN in the State of Iowa
· BSN preferred
· Minimum 2 years working experience with those with complex needs
· Knowledge of psychotropic medications and DMS-IV diagnosis
