JOB SUMMARY: Are you a detail-oriented and leadership-driven Registered Nurse looking to oversee clinical excellence? We are seeking a full-time MDS Coordinator to conduct and coordinate the Resident Assessment Instrument (RAI) process. In this critical role, you will be responsible for the implementation and ongoing evaluation of each resident’s comprehensive plan of care. RESPONSIBILITIES:

  • Coordinate the facility’s RAI process in accordance with all state and federal regulations.

  • Accurately complete all MDS assessments, supporting clinical documentation, and CAAs.

  • Schedule and lead interdisciplinary care plan team meetings to reflect each resident’s current needs.

  • Audit medical records to ensure supporting documentation is present for all items coded on the MDS.

  • Utilize the current RAI Manual to schedule Assessment Reference Dates (ARDs) for payment and OBRA assessments.

  • Transmit assessments frequently to the QIES ASAP System database and correct any fatal errors immediately.

  • Collaborate with health plans, physicians, and the IDT to ensure insurance approvals and eligibility criteria are met.

  • Provide education to department heads, physicians, and staff regarding the MDS process and coverage guidelines.

  • Participate in resource utilization and triple-check meetings prior to billing claims.

  • Assist with the admission, discharge, or direct care of residents as needed.

QUALIFICATIONS:

  • Current, unencumbered, active license to practice as a Licensed Practical Nurse or Registered Nurse in the state of Connecticut.

  • At least 2 years of experience in MDS preferred

  • Resident Assessment Coordinator (RAC) training completion required within six months of hire; RAC Certification preferred.

  • Intermediate computer skills, including proficiency in Word, Excel, PowerPoint, and electronic medical record systems.

  • Strong organizational, time-management, and independent decision-making skills.

  • Effective verbal and written communication skills in English.
