Licensed Practical Nurse (LPN)

As a Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering high-quality patient care to our residents. We are seeking a compassionate and skilled nurse to provide medical and rehabilitation services to our short-term and long-term care residents.

Licensed Practical Nurse (LPN) Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Document resident progress, report any changes or concerns to the healthcare team, and participate in interdisciplinary care conferences
• Collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes
• Maintain accurate and up-to-date medical records

Licensed Practical Nurse (LPN) Requirements:

• Current LPN licensure
• 0-2 years of experience in a post-acute care setting (new graduates welcome to apply)
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of a team
• Basic computer skills and familiarity with electronic medical records

All responsibilities are to be carried out in accordance with applicable state regulations. Responsibilities Provides general nursing services to patients under the supervision of a physician or other licensed provider within the scope of practice for the LPN. Performs clinical skills, such as phlebotomy, EKGs, point of care testing and medication administration. Assists with medical office procedures and initiating/discontinuing IV fluids as directed. Follows HIPAA, CLIA, OSHA, Medication Management and Infection Prevention procedures for cleaning and sterilization of equipment and/or instruments. Obtains vital signs, patient history and maintains accurate and prompt charting in the patient's record. Triage calls, provide test and lab results to patients and present relevant information to provider with correct medical terminology. BJC has determined this is a safety-sensitive position.
