RN MDS Full-time

We are currently seeking a detail-oriented and experienced MDS Coordinator to join our team!

Responsibilities:

  • Complete and oversee accurate and timely MDS assessments in accordance with federal and state regulations
  • Coordinate care plans and ensure compliance with Medicare/Medicaid guidelines
  • Collaborate with interdisciplinary team to optimize resident care and reimbursement
  • Monitor documentation and ensure accuracy for audits and reviews
  • Stay up to date with regulatory changes and MDS guidelines

Qualifications:

  • Active RN or LPN license
  • Previous MDS experience in LTC/SNF preferred
  • Strong knowledge of RAI process and care planning
  • Excellent attention to detail and organizational skills
  • Ability to work collaboratively with clinical staff
