RN Supervisor of Transitional Care - Case Management - Full Time - Days

Job Summary

The Case Management Supervisor provides oversight for the day to day operations of the Case Management department and provides education, clinical supervision and support to the Case Management team as needed.  Follows the mission, vision, values, policies and procedures of Mohawk Valley Health System. 

Core Job Responsibilities

  • Ensures that the Case Management team works effectively and efficiently with the interdisciplinary health care team to meet departmental and organizational goals.
  • Handle scheduling, distribute case assignments or instructions, and monitor work for quality, completeness, and client/patient satisfaction.  Handles call outs and PTO requests.  
  • Supervise assigned Discharge Planners and Nurse Case Managers within scope of license.   
  • Serves as a resource to Case Management, ensuring patients have safe, timely and appropriate discharge plans.  Oversees collaboration with MVHS sites and other facilities and services to arrange post-hospital care or appropriate services.
  • Monitors complex cases and assists to remove barriers to care.
  • Assists with identifying over-utilization of services and reports trends to Case Management leadership. 
  • Ensures that the Case Management team are adhering to Conditions of Participation and all regulatory requirements that impact patient rights. 
  • Ensures Case Management Team practices in accordance with New York State Nurse Practice Act, regulatory standards and MVHS policies and procedures.  
  • Provides service excellence to patients and their families/visitors that meets or exceeds patient expectations through offering empathetic and compassionate patient-centered care. 
  • Audits charts for trends.
  • Handles routine patient/family complaints and escalates appropriately.
  • With leadership and Human Resources guidance, provides input to or makes recommendations on employee hires, performance reviews, transfers, promotions, disciplinary actions, terminations and similar actions. Resolves grievances and other staff problems in a fair, timely and consistent manner also in partnership with HR.
  • Ensures all staff have the orientation, resources, tools, information and training necessary to fulfill their duties.
  • Promotes and role models teamwork and customer service in all interactions.
  • Keeps Manager and leadership updated on issues, problems and progress towards goals.
  • Takes on Case Management assignments when needed.
  • Covers operationally for the Supervisor, Social Work when needed.
  • Keeps current in best practices in Case Management.
  • Perform other duties as required.

Education/Experience Requirements

REQUIRED:

  • Bachelor’s degree or equivalent experience.
  • 3 years of experience in Case Management.
  • Excellent verbal, written and interpersonal skills.
  • Demonstrated job knowledge, professionalism and critical thinking skills to lead, guide and motivate others.
  • Proficient with MS Office and EHR (Epic).

PREFERRED:

  • Prior supervisory experience.

Licensure/Certification Requirements

REQUIRED:

  • RN licensure or eligible in NY State.

PREFERRED:

  • Case management certification (CCM, ANCC)
