What You'll Be Doing
The Case Manager, RN is responsible for the interdisciplinary planning, coordination, implementation and evaluation of health care services to ensure appropriate utilization review and discharge planning.
The successful candidate will be able to:
· Formulates a plan of care to meet identified needs in most appropriate setting at time of initial assessment and documents in medical records.
· Performs utilization review function within 24 business hours of patient admission to the hospital and documents same on internal worksheet.
· Interviews patient and/or family with risk criteria for education needs and
· any special discharge concerns and continues to follow through discharge.
· Actively participates interdisciplinary rounds and provides information to staff regarding InterQual criteria, payor guidelines and identified variances in the care plan.
· Identifies issues that require patient education prior to discharge.
What We're Looking For
- Degree in Nursing – BSN preferred
- Valid NJ RN
- Minimum 2-3 years’ experience in Healthcare setting preferred
- Milliman Guidelines, evidence-based clinical practice, that help determine the preferred course of treatment in medical situations.
- Successfully completes initial competency verification for specialty area.
- Demonstrated people/interpersonal skills.
- Ability to communicate clearly in English, both orally and in writing.
- Ability to handle confidential information.
