What You’ll Be Doing:

Act as the key liaison for residents and families regarding care coordination, discharge planning, and emotional support.
Facilitate admission and discharge processes, including assessments and transition planning.
Conduct psychosocial assessments and maintain ongoing documentation of resident needs.
Collaborate with interdisciplinary teams to develop and monitor individualized care plans.
Connect residents and families with appropriate community resources and support services.
Monitor and support the emotional well-being of residents through counseling, advocacy, and crisis intervention.
Ensure compliance with state and federal regulations, including resident rights and social service requirements.

Qualifications:

RN license in NV
Degree in Social Work, Human Services, Psychology, or a related field (LBSW or equivalent required).
Prior experience in a skilled nursing facility or healthcare setting strongly preferred.
Knowledge of Medicare/Medicaid processes, discharge planning, and community resources.
Excellent communication, organization, and interpersonal skills.
A heart for serving seniors and the ability to build trust with residents and families.
