Position Overview (MDS & Admissions Nurse Coordinator – Full-Time)
Heritage Grove Estates is seeking a highly skilled and detail-oriented Registered Nurse (RN) to serve in a dual-role capacity as MDS & Admissions Nurse Coordinator. This position plays a critical role in ensuring accurate clinical assessments, regulatory compliance, and optimal reimbursement outcomes, while also leading the admissions process to support seamless resident transitions.
In this role, you will oversee MDS completion, coordinate interdisciplinary care planning, and manage clinical admissions to ensure each resident receives high-quality, person-centered care aligned with CMS and KDADS regulatory standards.
Key Responsibilities
Conduct comprehensive clinical assessments for all admissions and readmissions, ensuring appropriate level of care and documentation
Coordinate with the MDS Director and interdisciplinary team to develop, review, and update care plans in accordance with regulatory requirements
Establish Assessment Reference Dates (ARDs) and ensure timely and accurate completion and submission of all MDS assessments
Maintain current knowledge of RAI Manual guidelines, CMS updates, and state regulatory changes
Transmit MDS data to CMS and state databases in compliance with all reporting requirements
Obtain and update qualifying diagnoses for residents receiving Medicare Part A services to ensure accurate reimbursement under PDPM
Collaborate with therapy services to monitor and report progress for Medicare Part A and B residents
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Respond to medical emergencies in accordance with established clinical protocols
Serve as a clinical resource to nursing staff regarding admissions processes, MDS completion, and documentation standards
Ensure accurate entry of admission and readmission data, including ICD-10 coding, physician orders, allergies, and resident demographics
Qualifications
Graduate of an accredited nursing program with a current, active Registered Nurse (RN) license in good standing in the State of Kansas
Current CPR certification
Strong working knowledge of CMS regulations, RAI/MDS processes, and skilled nursing facility compliance requirements
Experience with Medicare Part A/B reimbursement and PDPM methodology preferred
Demonstrated ability to provide compassionate, resident-centered care across diverse populations
Strong collaboration and teamwork skills within a multidisciplinary environment
Effective critical thinking and problem-solving abilities
Excellent communication and interpersonal skills
