Position Purpose:

The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.

What You Will Do:

  • Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
  • Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
  • Provides statistical/budgetary data to appropriate Palliative care program
  • Works collaboratively with all departments to ensure execution of quality delivery of services

Minimum Qualifications:

  • Graduate of an approved nursing education program.
  • Current Registered Nurse licensure in the state of practice.
  • CPR certification
  • Hospice/Palliative Care certification preferred.
  • Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,
  • Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills
  • Must be able to work independently and on a team
  • Ability to perform diversified duties with time limitations
  • Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care

Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.
