Position Overview

As a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care.

You’ll work independently in the field - but never alone. Our Clinical Supervisors and operational staff are readily available to support you.

Responsibilities

Deliver skilled nursing care in accordance with the established plan of care

Monitor and assess patient conditions during home visits or shifts

Communicate changes in patient status to the Clinical Supervisor

Provide patient and caregiver education regarding medications, treatments, and disease management

Accurately document patient care and clinical observations

Coordinate care with other healthcare team members as appropriate

Maintain compliance with agency and state guidelines
