
QAPI Registered Nurse Manager (RN) – Home Health & Hospice
The Family Home Health Network, a partner to the Alden Network, is a dynamic post-acute care service provider of intermittent home health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.
Position Summary
The QAPI Registered Nurse (RN) is responsible for leading, developing, and maintaining the Quality Assurance and Performance Improvement (QAPI) program across Home Health and Hospice services. This role ensures regulatory compliance, drives clinical excellence, and promotes a culture of continuous improvement. The QAPI RN manages a multidisciplinary team and collaborates with leadership to enhance patient outcomes, safety, and operational efficiency.
Key Responsibilities
Program Leadership & Oversight

Design, implement, and evaluate the organization’s QAPI program in alignment with federal, state, and accreditation standards.

Lead performance improvement initiatives using data-driven methodologies.

Develop annual QAPI plans, goals, and measurable outcomes.
Team Management

Supervise, mentor, and evaluate QAPI team members.

Delegate responsibilities and ensure timely completion of audits, reviews, and reports.

Foster a collaborative and accountable team environment focused on quality outcomes.
Regulatory Compliance

Ensure compliance with CMS Conditions of Participation for Home Health and Hospice.

Maintain readiness for surveys and accreditation reviews.

Interpret and communicate regulatory updates to clinical and administrative staff.
Clinical Quality & Auditing

Oversee clinical record reviews, audits, and documentation compliance.
-
Identify trends, gaps, and risks in patient care delivery.

Develop corrective action plans and monitor effectiveness.
Data Analysis & Reporting

Collect, analyze, and report quality metrics (e.g., OASIS, CAHPS, HIS).

Present findings to leadership and governing bodies.

Utilize dashboards and data tools to track performance indicators.
Education & Training

Provide ongoing education to clinical staff on quality standards, documentation, and best practices.

Support onboarding and competency validation related to quality measures.

Lead performance improvement training initiatives.
Interdisciplinary Collaboration

Partner with clinical leadership, operations, and compliance teams to drive improvement strategies.

Participate in case conferences, risk management meetings, and leadership committees.

Serve as a resource for quality-related inquiries and problem-solving.
Qualifications
Education & Licensure
-
Active Registered Nurse (RN) license in the state of practice.

Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred.
- Experience with HCHB EMR preferred.
Experience
-
Minimum of 3–5 years of clinical experience in Home Health and/or Hospice.

At least 2 years of experience in quality assurance, performance improvement, or clinical leadership.

Prior supervisory or team management experience required.
Knowledge, Skills, and Abilities

Strong understanding of QAPI principles, CMS regulations, and accreditation standards.

Proficiency in data analysis, clinical auditing, and quality reporting tools.

Excellent leadership, communication, and organizational skills.

Ability to manage multiple projects and drive results in a fast-paced environment.
