Home Health Licensed Practical Nurse (LPN)

Partner with an RN. Build real relationships. Deliver care that truly matters.

At Elara Caring, we believe the best care happens where patients feel most comfortable—at home. Every day, our teams support more than 60,000 patients across the country, and none of that happens without strong clinical partnerships.

As a Home Health LPN, you’re not working alone—you’re part of a dedicated RN-led care team. Together, you and your RN Case Manager collaborate, communicate, and support each other to deliver high-quality, personalized care that helps patients stay safe and independent at home.

If you’re looking for a role where your voice is heard, your skills are valued, and your work has a direct impact—this is it.

Why Join Elara Caring?

  • True RN + LPN Team Model – Work side-by-side with an RN Case Manager who supports, guides, and collaborates with you

  • Autonomy with Support – Manage your visits independently while having real-time clinical backup

  • Career growth opportunities within a national organization

  • Comprehensive benefits: medical, dental, vision, 401(k) with match

  • Generous PTO, holidays, and family/pet bereavement leave

Your Role as an LPN on the Elara Team

  • Partner closely with your RN Case Manager to deliver care aligned with each patient’s individualized Plan of Care

  • Provide skilled nursing visits, reinforcing care plans and patient goals in the home setting

  • Communicate changes in condition, concerns, and updates to your RN partner to ensure coordinated, proactive care

  • Contribute your clinical insight—your observations help shape and improve the Plan of Care

  • Educate patients and caregivers on medications, treatments, and self-care techniques

  • Help prevent avoidable hospitalizations through early intervention and patient support

  • Maintain accurate, timely documentation of visits and care provided

  • Build trust with patients and families through compassionate, respectful care

What You Bring

  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

  • Current State License as a Licensed Practical Nurse (LPN/LVN)

  • At least 1 year of clinical experience

  • Home health or hospice experience preferred, but not required

  • Willingness to travel within the assigned territory

  • Physical Requirements: Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.

Why This Role Stands Out

This isn’t just task-based nursing. It’s relationship-based care, backed by a true clinical partnership model. You’ll have the independence to manage your day—plus the confidence of knowing your RN partner is right there with you when it matters.
