Job Summary:
A well-established and highly regarded post-acute care organization is seeking an experienced MDS Coordinator (RN) to join its clinical leadership team. This is a confidential search for a key role responsible for ensuring the accuracy, compliance, and integrity of resident assessments and care planning.

Key Responsibilities:
Complete accurate MDS assessments and comprehensive care plans
Initiate and oversee care planning processes and related activities
Develop and distribute monthly care plan schedules in a timely manner
Maintain and update all clinical assessments in accordance with regulations
Conduct ongoing audits of clinical documentation for accuracy and compliance
Partner with nursing leadership to address and resolve documentation issues
Ensure strict confidentiality of all resident and organizational information
Requirements:
Prior MDS 3.0 experience required
Active New Jersey RN License
Previous Long-Term Care experience required
Strong organizational and time management skills
Proficiency with clinical and documentation systems
Excellent communication and collaboration abilities
Team-oriented with a professional demeanor
