SUMMARY OF JOB A Hospice Visiting LPN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses under the guidance of the Hospice RN. The LPN functions as a member of the health care team and provides support to meet the needs of the patient, family, and community. SUMMARY OF JOB A Triage RN receives and prioritizes all incoming calls for the outpatient oncology practice in order to continue delivery of safe, holistic care to the oncology and hematology patient population. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provides appropriate support including education of the disease process to the patient and family.
