Responsibilities

🩺 Conduct comprehensive resident assessments and ongoing evaluations
📋 Develop, implement, and update individualized care plans
💊 Administer medications, treatments, and IV therapies per physician orders
👀 Monitor resident conditions and respond to changes in health status
💻 Accurately document care, assessments, and outcomes in the EHR
🤝 Coordinate care with physicians and the interdisciplinary team
🗣️ Communicate effectively with residents and families
👩‍⚕️ Supervise and support RN's and CNA/GNAs; delegate tasks appropriately
✅ Ensure compliance with state, federal, and CMS long‑term care regulations

Qualifications

Full Time, Full-Time Flex, Part Time and PRN
Schedule: Days, Evenings and Nights
Every other weekend availability (Saturday and Sunday)
Maryland LPN license, valid and in good standing
