POSITION SUMMARY:
To be fully engaged in providing Quality/No Harm, Customer Experiences, and Stewardship by demonstrating leadership through the assessment, planning, implementation and evaluation of patient care according to the standards of professional nursing practice. Nursing care is provided to the individual patient, based upon cultural, spiritual and age specific needs.
PRIMARY ACCOUNTABILITIES:
Engagement:
- Associate supports engagement through adherence to the I-CARE commitments.
Quality/No Harm:
- Perform complete comprehensive Assessment at every patient visit, to include but not limited to psychosocial, safety, and risk factors.
- Demonstrates effective medication administration provided to patients as evidenced by Field Supervisory/Infection Control Evaluations, and Clinical Practice.
- Assist Clinical Team with performing accurate Oasis Assessment time points such as SOC, ROC, Recerts, Transfers and Discharges when necessary, to ensure quality patient outcomes. .
- Demonstrates effective communication with other members of the Clinical Team and promotes a cohesive multi-disciplinary approach to patient care.
- Utilizes Health First Home Care computer systems with regards to all aspects of communication and documentation as set forth by Agency Policies, MCR/JCAHO requirements for patient care in the home setting.
- Adheres to all Health First, Inc. and all Health First Home Care policies and procedures.
- Demonstrates effective and safe clinical practice as evidenced by Field Supervisory/Infection Control Evaluations, completion of yearly Competency Assessment, and Age Specific Checklist.
- Documents all patient care, exacerbation or progress toward goals as set for patient episode of care, physician notifications and referrals to meet patients’ needs outside of Agency scope of practice.
- Demonstrates professionalism by exhibiting dependability, reliability and flexibility with a focus on a Team Approach to patient care.
- Demonstrates commitment to the clinical Team with regard to maintaining continuity of care within that Teams patient census.
- Assists with the Orientation process of new associates and staff education.
- Complies with State and Federal Regulations, JCAHO standards in reference to delivery of Home Care Services.
- Actively participates in Quality Improvement processes, including peer chart reviews and self chart reviews.
- Supervises the Home Health Aide at least every 14 days, and supervises the Licensed Practical Nurse (LPN) at least every 30 days. Communicates effectively with discipline to provide continuity and comprehensive care.
- When needed, assists the Weekend Staff for coverage to maintain continuum of care.
Customer Experience:
- Meets or exceeds Press Ganey Patient Satisfaction goals as established every year § Provides patient, family, and/or caregiver education as directed by the plan of care.
- Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences.
- Contributes to the patient experience as evidenced by positive patient satisfaction scores and HHCAHPs scores for department or written compliments by patients/others as well as consistent performance of purposeful skilled RN Home Visits.
- Informs patient/family of visit schedule in advance by calling the night before and/or if schedule changes.
- Performs other related duties as assigned or requested.
Stewardship:
- Uses and cares for equipment and supplies in a cost-effective manner.
- Meets productivity standards.
Work Experience
QUALIFICATIONS REQUIRED:
- Graduate of an approved school of professional nursing.
- Current Florida RN licensure or endorsement.
- Successful completion of all post-hire requirements, including, if applicable for role, Level II background screening/fingerprinting.
- Current American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Certification.
- Maintain all educational requirements outlined by regulatory entities and the organization.
- Proficiency in MS Office, electronic record and other systems and software relevant to role.
- Current Florida Driver’s License, automobile insurance and reliable transportation.
- Must have 1 year of acute care nursing experience.
- 1 year of homecare experience preferred.
- IV Therapy and phlebotomy experience required.
- Able to speak/understand written and verbal instructions in the English language.
- Must be available between 8am -5pm for patient care and willing to be available and flex hours to accommodate scheduled after hours visits when necessary.
- Will rotate on-call hours as assigned.
- Will be expected to work one minor and one major Holiday each year per HFHC Policy.
- Will comply with Hurricane Preparedness Policy.
- Will be required to attend staff meetings a minimum of 80% of the time.
PHYSICAL DEMANDS:
- Visual acuity and hand-eye coordination to perform clinical tasks; Corrected vision and hearing to normal range.
- Able to lift up to 40 pounds unassisted.
- Stand, walk, bend, and stoop: sit for long period of time on computer.
- Potential for exposure to bio-hazardous waste and known or unknown diseases.
MENTAL DEMANDS:
- Work in often stressful environment with patients/families at various ages and levels of understanding.
- Concentrate on multiple priorities.
- Respond appropriately to life threatening situations.
- Critical thinking and problem-solving skills.
