Job Description Summary:

Pay Range: $62.78 - $78.47 - $94.16

The Nurse (RN) – Navigator, Cardiology streamlines the patient journey from diagnosis through treatment, serving as the primary point of contact to ensure seamless, evidence-based care. They perform nursing assessments, triage, educate patients, coordinate appointments, address barriers to care and facilitate multidisciplinary communication to improve outcomes. The position involves direct contact and communication with patients — in person, by phone, or through the patient portal.

Job Requirements, Prerequisites and Essential Functions:

Essential Functions and Responsibilities: 

Individual:

  • Guides patients through complex cardiovascular care pathways (e.g., structural heart, heart failure, women’s heart health).

  • Acts as a liaison between cardiologists, primary care providers, and the interdisciplinary team.

  • Triages patients’ physical symptoms / arrhythmias appropriately and consults with cardiologists per protocol.

  • Participates in Transition of Care Management to ensure discharge medications and instructions are understood by patient and/or family members. Updates medication and problem list in the Electronic Health Record and ensures follow-up with cardiologists.

  • Educates patients and families on cardiac conditions, medications, and lifestyle modifications.

  • Oversees the gathering of patient information and records from healthcare providers in preparation for appointment. Ensures that subsequent medical information is added to patient chart as necessary.

  • Refers patient to appropriate staff regarding advance directives, community resources,   transportation and insurance issues.

  • Facilitates access to medical team and informs physician of status changes or obstacles to patient treatment compliance.

  • Reviews diagnostic results, manages caseloads, and tracks patient progress, particularly in reducing readmission rates.

  • Takes initiative to identify and provide solutions to work-related problems, continuously seeking ways to improve services.

  • Updates Clinical Leader of changes affecting their schedule

Team:

  • Promotes C.A.R.E.S. standards within the MarinHealth organization.

  • Assists in maintaining safety standards within the clinic.

  • Supports program development in collaboration with Cardiology Leadership and key Cardiologists.

  • Coordinates care with the multi-disciplinary care team including need for interpreter services and facilitates referrals as needed. 

  • Tracks data in Population Health management system with EPIC (APeX) and develops care pathways that support patient’s healthcare journey.

  • Maintains and helps to ensure that all documentation is in a state of audit-readiness.

  • Promotes ongoing communication and collaboration with the clinical leader, physicians, and appropriate staff to ensure patient safety and adherence to clinic protocols.

  • Communicates with internal and external referrals to ensure prompt patient access.

Practice:

  • Professional representative within the clinic and the community.

  • Participates in clinic meetings as required.

  • Demonstrates positive initiative and cooperation between departments.

  • Other duties as assigned.

Qualifications:

Education:

  • Graduate of an accredited baccalaureate school in a healthcare-related field.

License & Certifications:

  • Current Registered Nurse California license.

  • BLS Certification (American Heart Association) required.

Experience:

  • Experience working in the ambulatory clinic healthcare setting.

  • Ability to work independently in a fast-paced, demanding environment with minimal supervision; ability to manage and oversee multiple tasks simultaneously, including high daily call volume.

  • Excellent oral and written communication skills.

  • Diverse and culturally sensitive customer service skills.

  • Ability to manage high stress, chronically ill population with complicated psycho-social issues.

  • Computer skills- Microsoft Word, Excel

  • Two-year navigation experience in Cardiology and/or Women’s Health programs preferred.

  • Prior experience leading groups of clinical and non-clinic staff for care coordination preferred.

  • Experience working with healthcare payor systems preferred.

Overview SCU (Surgical Care Unit) RN is responsible for the preoperative preparation and recovering of all surgical patients. Overview This position is responsible for the daily work and work flow within the Surgical Care Unit (SCU). The Clinical Coordinator/SCU Charge RN supervises the nursing staff in coordinating schedules, nursing assignments and patient care in the preoperative and postoperative phases of surgery from preadmission through discharge. Responsibilities Daily staffing: Assigns nursing staff according to caseload, types of surgery cases, and staffing needs in accordance with Title 22.
