RN MDS Full-time

Must have prior Nursing Home experience! We are currently seeking a dedicated MDS RN to join our team as Director of the department. The RN will play a crucial role in ensuring accurate and timely completion of Minimum Data Set assessments for our patients. ** Requirements: Active RN NYS Lic /  Prior Nursing home experience

**Responsibilities:**

- Conduct comprehensive assessments of patients' medical records and care plans

- Collaborate with healthcare professionals to ensure accurate coding and documentation

- Utilize knowledge of ICD-10 coding and medical terminology to accurately assess patient needs

- Coordinate with interdisciplinary teams to develop individualized care plans

- Ensure compliance with HIPAA regulations and maintain patient confidentiality

- Assist in discharge planning and coordination of patient care transitions

- Manage medical records efficiently and accurately

- Assist in medical coding tasks related to patient diagnoses and procedures

**Qualifications:**

- Bachelor's degree in Nursing or related field preferred

- Proficent in MDS 3.0 assessments, scheduling , medicare guidelines and have strong organizational skills.

- Familiarity with electronic medical record systems

- Excellent communication skills to interact effectively with patients, families, and healthcare professionals

- Ability to prioritize tasks and work effectively in a fast-paced environment

- Knowledge of discharge planning processes and regulations
