Position Overview:

The Clinic Registered Nurse (RN) is responsible for the care of clinic patients. The Clinic RN is skilled in all aspects of nursing process, delegates, and supervises staff in assistive roles on the same shift and is responsible to deliver the highest of customer service standards for patients and families presenting for services at Winona Health. The Clinic RN will assess and triage patients to make certain of the appropriateness of care. Will provide patient care including but not limited to rooming of patients, performing assessments and gathering of data for the physician. Patient information will be entered into the Electronic Medical Record (EMR). Direct patient care will include but is not limited to taking vital signs, medication administration, treatments, intravenous therapy, monitoring of patients, scheduling and coordination of care for referral to specialty care and general assistance to the provider as needed. Other responsibilities include assisting with procedures. Must be able to assist at all Winona Health Clinic locations if necessary.

Essential Duties & Responsibilities:

Specific to Women’s Health RN:

  • Perform non-stress tests to pregnant patients as ordered.
  • Provide first & third trimester education.
  • Oversee LPNs in compliance with the Nurse Practice Act.
  • Comprehensive assessment of patients.
  • Triage presenting patients for appropriateness of care. 
  • Take vital signs and monitor patients.
  • Administration of medications, intravenous therapy, and immunizations as ordered.
  • Implement protocols/orders after appropriate assessments.
  • Assist with surgical procedures, monitoring and assessments of sedated patients.
  • Insertion and discontinuation of foley catheters.
  • Entering all applicable data into the EMR (Cerner).
  • Contact patients with lab results and medication changes as ordered by the physician/provider.
  • Coordinate schedules, procedures and supplies.
  • Coordinate and schedule care for patients referred for specialty care.
  • Scheduling of surgery or diagnostic studies.
  • Coordinate nurse schedule with physician schedule in clinic and/or operating room (OR).
  • Ensuring nursing supplies and medications are properly ordered and available.
  • Other duties as assigned.

Required:

  • Associate’s Degree or Bachelor’s Degree in Nursing
  • RN License - MN
  • Current BLS Certification
  • Basic computer skills: Word, Excel, Outlook, and Patient Information Database (Cerner)

Preferred:

  • Knowledge of basic fetal monitoring

Winona Senior Services Lake Winona Manor Infection Control and Assessment Nurse (1.0 FTE) 80 hours a pay period Days, 7:30am-5pm Weekends: Every 4th Weekend Holidays: One major, one minor Position Overview: The Infection Control (IC) and Assessment Nurse supports long-term care residents by facilitating appropriate assessment, evaluation and action needed to improve quality of care according to evidenced-based practices. This individual identifies risks and incorporates appropriate approaches, documentation and monitors of processes and care. The IC and Assessment Nurse works closely with teams on each unit of Lake Winona Manor as a mentor and facilitator for the delivery and execution of the care plan process. Essential Duties & Responsibilities: Performs thorough and timely admission, quarterly, and other MDS-related assessments as required by the State of Minnesota. Evaluate the resident holistically to identify risks, strengths, and needs and collaborates to develop individual care plans especially on admission and during the care of wounds and infections. Participates on the Winona Health Committee as a representative of senior services. Performs ongoing analysis of infections, treatment and drives mitigation and prevention strategies. Tracks resident and caregiver infections, organisms and develops reports to present to members of the care team including the Quality Assurance & Improvement Committee. Participates as a member of the Quality Assurance and Improvement Committee and uses LEAN Methodology to make improvements daily within the department. Evaluates and provides complete documentation for open areas/wounds of residents on a weekly/prn basis. Documents the effectiveness of wound care treatments, makes modifications and collaborates with Part B Wound Care services in the facility. Ensures the plan of care is visible and adhered to by the interdisciplinary team. Conducts audits on care plan adherence Ensures documentation for ongoing and acute issues is appropriate, complete, and timely. Provides support and guidance to staff in the identification of significant changes in resident needs and services. Identifies gaps and supports needed to support successful discharges for short-term residents. Understand the Nursing Practice Process including assessment, implementation, planning, and evaluation of resident care. Keeps current on nursing theory, research, medications, and legal issues related to resident-based services. Assists with other nursing duties and performance improvement projects as assigned (i.e. vaccine administration, Covid-19 outbreak testing, Kaizens/CIP event teams). Provides care and supervision of caregivers and residents at the level of licensure. Provides nursing services to residents in accordance with facility policy and professional standards of care.
