Job Description

When in the Nurse Navigator portion of the job:

  • Acts as a liaison between the patient, their families, and internal and external providers.
  • Establishes a strong rapport with patients, families and other members of the health care team and community.
  • Assists with the implementation of a treatment plan for patients being discharged from the department.
  • Facilitates appointment scheduling, referral process and providing clear discharge education to the patient and family when appropriate.
  • Recognizes barriers to healthcare and connects patients to the resources, services, and system providers during discharge.
  • Communicates with family members (next of kin) of impending patient discharges to expedite transport back to a facility or home.
  • Facilitates external transport to assist with more timely transport back to facilities.
  • Collaborates with ED Ambassadors, Patient Liaisons and the multidisciplinary team on Patient and Family Communication and Experience.
  • Daily review and triage of all boarding patients in the department, coordinating bed needs by hours boarding and clinical needs.
  • Daily review of our geriatric population as well as our medically compromised patients in the emergency department for expedited placement.
  • Ensures the highest quality customer service for patients and providers.
  • Facilitates teamwork and multidisciplinary care for patients, staff, and facility.
  • Documents all work in the EMR.
  • Works to identify technology that will assist with patient flow and patient experience.
  • Conducts discharge phone calls as workload allows.
  • Facilitates transport Request for patients with available in-patient beds
  • Maintains data related to discharge times, trends and patient satisfaction related to discharge process.


When in the ED Registered Nurse Portion of the role:

  • Promotes and restores patient’s health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members.
  • Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Promotes patient’s independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; and answering questions.
  • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; and following hospital and nursing division’s philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
  • Resolves patient problems and needs by utilizing multidisciplinary team strategies.
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations; and calling for assistance from health care support personnel.
  • Protects patients and employees by adhering to infection-control policies and protocols; medication administration and storage procedures; and controlled substance regulations.
  • Documents patient care services by charting in patient and department records.
  • Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
  • Maintains patient confidence and protects operations by keeping information confidential.
  • Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer’s instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; and evaluating new equipment and techniques.
  • Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; and using equipment and supplies as needed to accomplish job results.
  • Maintains professional and technical knowledge by attending educational workshops and participating in professional societies.
  • Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods.
  • Contributes to team effort by accomplishing related results as needed.


Qualifications

QUALIFICATIONS

  • Active RN license / BSN preferred
  • 3-5 years of experience in an emergency room setting.
  • Able to work independently and be self – motivated to meet and assist patients in resolving complex care issues related to their ED experience.
  • Ability to communicate clearly and effectively with a diverse population of patient, family, significant other and providers at all levels.
  • Strong interpersonal skills and the ability to function effectively in a critical environment
  • Ability to work on projects and collect pertinent data that will optimize operational efficiency and improve the patient experience.
  • Demonstrates flexibility and the ability to manage multiple complex situations
  • Demonstrates an ability to function in a goal driven manner.
  • Understanding of the cultural aspects that impact health care in the emergency room setting.


Skills

  • Strong knowledge of EPIC workflow and functional ability.
  • Strong Microsoft office skills
  • Understand of MyChart application to assist patients and families with obtaining information regarding their clinical findings.
  • Willingness to learn In-patient / outpatient scheduling workflow to assist with patient appointments.
  • Must have knowledge of HIPAA guidelines, PHI privacy requirements, community resources and transportation modalities
  • Able to create monthly data reports to monitor all ED patient flow metrics.
