
Hospice Admissions Nurse
Emunah Hospice of King David is seeking a new Admissions Nurse to join our growing Hospice Team.
We are very proud of our Emunah Hospice program which serves a majority of patients on our Beachwood King David campus as well as patients residing in the community within a limited range.

A hospice admissions nurse is a registered nurse responsible for evaluating patients for hospice eligibility, coordinating the admission process, and ensuring a smooth transition into hospice care.
Key Responsibilities
1. Patient Assessment & Eligibility
Conduct comprehensive physical, psychosocial, and emotional assessments
Determine eligibility for hospice based on criteria (often guided by Centers for Medicare & Medicaid Services regulations)
Collaborate with physicians to confirm prognosis (typically 6 months or less)
2. Admission Process
Explain hospice philosophy, services, and goals of care to patients and families
Obtain informed consent and complete admission paperwork
Initiate the plan of care and coordinate with the interdisciplinary team
3. Care Coordination
Communicate with doctors, case managers, hospitals, and families
Arrange for equipment, medications, and initial visits
Ensure continuity of care during the transition
4. Documentation & Compliance
Maintain accurate, timely clinical documentation
Ensure compliance with regulatory standards (Medicare, state laws, agency policies)
5. Patient & Family Education
Educate on symptom management, disease progression, and what to expect
Provide emotional support during a difficult transition
Qualifications
Active RN license
1–2+ years of clinical nursing experience (often hospice, palliative care, ICU, or med-surg preferred)
Strong assessment and communication skills
Ability to handle sensitive conversations with empathy
Work Environment
Home visits, hospitals, nursing homes, or hospice facilities
Flexible or irregular hours (admissions can be urgent)
