Job Summary:

We are seeking a dedicated and compassionate LVN/RN to join our team at Granbury Rehab & Nursing, a premier skilled nursing facility in Granbury, Texas. As a member of our care team, you will be responsible for providing high-quality nursing care to our residents, promoting their independence, and supporting their overall well-being.

Essential Job Duties and Responsibilities:

  • Provide nursing care to residents as prescribed by physicians and in accordance with nursing practices and regulations

  • Prepare and review medical records and nurse's notes, and assess residents to observe and evaluate their physical and emotional status

  • Monitor resident activity, evaluate nursing care in accordance with professional standards, and provide nursing care according to physician's orders, care plans, established standards, and facility policies

  • Coordinate and assist or prepare for new admissions, transfers, and discharges of residents

  • Transcribe physician's telephone orders and communicate with residents, family members, other interdisciplinary team members, and management regarding resident status

Other Job Functions:

  • Assist and participate in developing and implementing a written care plan for individual residents

  • Participate in in-service training, assist in implementing orientation programs, and maintain professional competence

  • Monitor environment and care practices to ensure compliance with established safety, fire, and disaster protocols, as well as infection control and cleanliness standards

Physical and Sensory Requirements:

  • Works in a climate-controlled and well-lighted environment, with occasional interruptions, noise, and odors

  • Requires walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing, and pulling, as well as ability to distinguish smells, tastes, and temperatures

Qualifications:

  • Must be a Registered Nurse (RN) in good standing and currently licensed by the State

  • Experience as a Charge Nurse or supervisory experience preferred

  • Must be CPR Certified

  • Ability to convey, explain, or interpret complex specialized information related to long-term care rules and regulations

  • Strong verbal and written communication skills are required

  • Ability to complete continuing education hours as required by the State

Assist with special procedures, as appropriate in specialty clinics or on the units. Document inpatient record and other required forms such as Intake and Output, TPR graphic, and nursing notes. Use computerized information systems as required. Reinforce patient education and discharge instructions. Use and maintain simple and complex diagnostic tools and equipment (e.g., stethoscope, blood pressure cuff, cardiac monitor, digital readout, hemodynamic monitoring systems, infusion pumps, central venous lines, respiratory ventilator) to obtain and/or validate observations and provide for the care of all patients. Recognize and respond to serious alterations in patient's condition, determine appropriate action to include notification of the Registered Nurse/Physician, airway management and initiation of CPR. May be assigned or rotated between inpatient wards, clinics, and specialized units.
