Description
The Discharge Planner is responsible for identifying patient needs and priorities for care through assessment, planning, intervention, and evaluation. The Discharge Planner coordinates care between professional disciplines through documentation and oral communication as needed. The Discharge Planner will provide psychosocial support to patients and families so they can cope with chronic, acute, or terminal illness. Discharge Planner may advice family caregivers, counsel patients, and help plan for discharge needs.
Requirements
Current Idaho RN License or Current Idaho LPN License OR BSW/MSW licensed in the State of Idaho
Current BLS certification.
