Responsibilities:

  • Provide nursing care within the scope of licensure for assigned patients according to established nursing standards and hospital policies and procedures

  • Administer medication, perform treatments, and patient care responsibilities as prescribed

  • Consult with and keep MD/NP informed of activities, symptoms, reactions, and changes in the patient condition

  • Provide appropriate pain management techniques and educate patients and families regarding pain management

  • Administer medication following the 10 rights of medication administration within scope of practice

  • Assist with the rehabilitation and treatment of patients in accordance with the medical plan of care

  • Update and revise plan of care under the supervision of MD/NP

  • Assist physicians with examinations, treatments, and special procedures

  • Perform services requiring technical and manual skills as directed by MD/NP

  • Treat patients and their families with respect and dignity

  • Identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families

  • Interact professionally with patient/family and involve patient/family in the formation of the plan of care

  • Assist with admitting, discharging, and transferring of patients

  • Identify and address psychosocial needs of patients and family; communicate with Social Services/Discharge Planner regarding both in-hospital and post-discharge needs

  • Medication Administration & Management

    • Administer medications per physician/NP orders

    • Manage IV, SQ, SL, rectal, transdermal, and other hospice-related medication routes

    • Titrate comfort medications within ordered parameters

    • Monitor for side effects, adverse reactions, and effectiveness

    • Ensure timely availability of emergency comfort medications

    • Coordinate pharmacy needs with hospice provider

    Coordination with Hospice Interdisciplinary Team (IDT)

    • Communicate patient status changes to hospice physician/medical director

    • Collaborate with:

      • Hospice aides

      • Social workers

      • Chaplains

      • Nurse practitioners

      • Facility nursing staff

    • Participate in hospice IDT discussions and care planning

    • Coordinate transfers, discharge planning, or level-of-care changes if symptoms stabilize

    Documentation & Regulatory Compliance

    • Document assessments, interventions, and patient response thoroughly

    • Support documentation demonstrating:

      • Need for continued GIP level of care

      • Uncontrolled symptom burden

      • Frequent nursing interventions

    • Complete required hospice and SNF documentation timely

    • Ensure compliance with:

      • CMS hospice regulations

      • State nursing regulations

      • Facility policies

      • Hospice Conditions of Participation

    Family & Patient Support

    • Educate family regarding:

      • Disease progression

      • Dying process

      • Medication purpose/effects

      • Comfort-focused goals

    • Provide emotional support during crisis situations

    • Assist with difficult decision-making discussions

    • Support cultural and spiritual preferences

    • Offer bereavement-related support and referrals

    Skilled Nursing Interventions

    • Frequent reassessment and bedside monitoring

    • Wound and skin care as appropriate

    • Foley catheter management

    • Oxygen therapy management

    • Suctioning and secretion management

    • Fall prevention and safety monitoring

    • Positioning and pressure injury prevention

    • Infection prevention practices

Requirements:

  • RN

  • Ability to work varying shifts, including days, nights, and weekends
