About the Role:

As a Licensed Practical Nurse (LPN) with Axiom Gardens of Flora, you will play a vital role in our healthcare team, providing exceptional care to our residents in a fast-paced and dynamic environment. You will be responsible for:

  • Providing direct and indirect patient care, including administering medications, performing vital signs, and maintaining accurate records

  • Assisting with daily living activities, such as bathing, dressing, and grooming

  • Collaborating with other healthcare professionals to develop and implement individualized care plans

  • Identifying and reporting changes in resident condition to the nurse supervisor and other healthcare professionals

  • Maintaining a clean, safe, and organized environment in the resident's living space

Responsibilities:

  • Conduct initial and ongoing assessments of resident needs, and develop and implement individualized care plans

  • Monitor and report changes in resident condition to the nurse supervisor and other healthcare professionals

  • Provide emotional support and companionship to residents and their families

  • Participate in interdisciplinary team meetings to discuss resident care and progress

  • Maintain accurate and up-to-date records of resident care and services

  • Collaborate with other healthcare professionals to provide comprehensive and coordinated care
