Responsibilities:

Provide direct patient care, including administration of medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Demonstrate excellent communication and interpersonal skills to ensure effective communication with residents, families, and healthcare professionals
Participate in quality improvement initiatives and maintain accurate and up-to-date medical records
Collaborate with the nursing team to ensure continuity of care and optimal outcomes
Perform other duties as assigned by the Registered Nurse Manager or Director of Nursing
