Key Responsibilities:
- Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
- Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
- Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
