Qualifications   

  • Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
    • Compact licenses must be transferred to your state of residence within 90 days   
  • Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.
  • TB skin test (current within last 12 months)  
  • Must have reliable transportation

Informs and sets up patients and/or guardian for My Chart at the time of service and informs and educates patients regarding the benefits of signing up for My Chart. Provides patient teaching and supportive services as needed. Provides treatments as ordered.
