About the Role:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

  • Build strong relationships with the facility and maintain a patient-centered approach

  • Conduct on-site visits and provide remote care to residents

  • Document patient visits and maintain accurate records

  • Collaborate with other healthcare professionals to provide comprehensive care

  • Participate in Continuous Quality Improvement (CQI) initiatives to enhance patient outcomes

Requirements:

  • Active NP or PA license in the state of practice

  • 1+ year of experience in:

    • Geriatric care

    • Documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Masters degree in Nursing or Medical Education
