Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

Key Responsibilities:

• Administer medications and treatments as prescribed

• Monitor residents’ conditions and report changes to the care team

• Provide direct nursing care in accordance with care plans

• Supervise and support CNAs in delivering quality care

• Maintain accurate and timely documentation

• Ensure compliance with all clinical policies and regulatory standards

• Communicate effectively with residents, families, and team members

What You Bring

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)

What You'll Do Assesses and evaluates nursing care provided by clinical staff; (were policies/processes followed, documentation completeness, areas of risk, etc.) identifies areas for improvement, problem solves and communicates issues with the Group Health Center Manager. Knowledgeable of rules, regulations, Premise Health policies and procedures and ability to communicate effectively to team members as necessary. Participate in conferences and meetings as necessary. Participates in pertinent in-service education planned by organization to improve job performance and other meetings as designated by the Group Health Center Manager. Creates orientation packet and clinical training checklists for new employees and any identified re-training needs from Premise Central; then devotes time working with the individual to orient, retrain, coach, correct or educate employees.
