Position Summary
The Employee Health Nurse (LPN) is responsible for coordinating and maintaining the employee health program within the Skilled Nursing Facility (SNF). This role supports infection prevention, occupational health compliance, staff wellness, immunization tracking, workers’ compensation coordination, and regulatory compliance in accordance with New York State Department of Health (NYSDOH), OSHA, CDC, and facility policies.
The Employee Health Nurse works collaboratively with Human Resources, Infection Prevention, Nursing Administration, and department managers to ensure a safe and healthy work environment for all employees.
Essential Duties and Responsibilities
Employee Health & Occupational Services
· Conduct pre-employment health screenings and onboarding requirements for new hires.
· Review and maintain employee medical records, immunization records, TB testing, fit testing, and annual health assessments.
· Monitor compliance with required vaccinations, including influenza and other NYSDOH mandates.
· Coordinate annual tuberculosis screening/testing programs.
· Maintain employee exposure records and follow-up related to bloodborne pathogens and communicable diseases.
· Assist with return-to-work clearances following illness, injury, FMLA, or workplace injury.
· Provide staff education regarding infection prevention, workplace safety, and employee wellness initiatives.
Regulatory Compliance
· Ensure compliance with:
o New York State Department of Health (NYSDOH)
o OSHA regulations
o CDC infection control guidelines
o CMS requirements
o Facility policies and procedures
· Maintain accurate documentation and prepare reports for audits and surveys.
· Track mandatory annual employee requirements and certifications.
Infection Prevention Support
· Collaborate with the Infection Preventionist regarding communicable disease monitoring and outbreak management.
· Assist with staff exposure investigations and follow-up.
· Participate in infection control education and surveillance activities.
Administrative Responsibilities
· Maintain confidential employee health files in accordance with HIPAA standards.
· Prepare monthly and annual compliance reports.
· Schedule employee health appointments and follow-up visits.
· Participate in quality assurance and performance improvement activities.
Qualifications
· Current and active Licensed Practical Nurse (LPN) license in New York State required.
· Minimum one (1) year nursing experience required.
· Long-term care, skilled nursing facility, occupational health, or employee health experience preferred.
· Knowledge of infection control practices and OSHA regulations preferred.
· Strong organizational, communication, and documentation skills.
· Ability to maintain confidentiality and professionalism.
· Proficient in electronic medical records (EMR) and Microsoft Office applications.
Physical Requirements
· Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed.
· Ability to wear appropriate PPE and comply with infection control requirements.
· Ability to respond to emergencies and employee health situations promptly.
Work Environment
· Skilled Nursing Facility (SNF) / Long-Term Care setting.
· Frequent interaction with employees, residents, visitors, and healthcare professionals.
· Exposure to communicable diseases and healthcare-related occupational hazards.
Reporting Structure
Reports to:
· Director of Nursing (DON)
Schedule
· Full-Time
· Monday–Friday, with occasional flexibility based on facility needs and outbreak response.
