About the unit

We are able to provide care for long term care residents and post-acute rehab patients. Nurse-patient ratios vary depending on the shift and the population you will be serving. Each designated unit focuses on patient specialties, such as general rehab, neuro rehab, traumatic brain injuries and cancer. Cedar Street offers various programs for residents to participate in including music therapy, expressive arts and a garden room.

About Rehab and Nursing Center Cedar

The Rehabilitation & Nursing Center – Cedar was designed with the word “home” in mind. The home is more spacious than the previous 52-year-old Fuller Street facility and boasts many new amenities, including four spacious interior courtyards; a new dining room; on-site dialysis den; art room, private and semiprivate rooms with their own window, restroom and shower; modern furnishings; and new technology, to make these community members feel at home.

Scope of work 

Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team. Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit. Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding. Documents care provided as directed in a manner consistent with guidelines.

Qualifications

  • High School Diploma or equivalent
  • Completion of State of Michigan approved CNA Training Course
  • CRT-Nursing Assistant - Michigan Department of Licensing & Regulatory Affairs, Bureau of Community and Health Systems within 90 days
  • AHA or ARC Basic Life Support (BLS) within 90 days

Patient Care Technician - Ortho Trauma Med-Surg * Days - 36hrs/wk

General Summary: Under the direction of a Registered Nurse (RN), the Patient Care Technician (PCT) provides basic nursing care to patients and performs related tasks necessary to support the unit. Requirements: Minimum High School Diploma or GED required. Must meet one of the following requirements: Current Certified Nurse Aide (CENA) certification, or Current enrollment (and continued progression) in a health science program required with completion of nursing fundamentals course and/or one clinical rotation preferred. Equivalent enrollment considered will include but is not limited to: Nursing School, Pre-PA School, Medical School, or closely related health science program, or Equivalent experience. Equivalent experience considered will include but is not limited to: Medical Assistant, Paramedic, Medical First Responder, EMT, Nurse Tech, Resident Aid/Assistant, or LPN. Basic understanding of Medical Terminology, Anatomy, Physiology, and Patient Confidentiality is preferred. Current Basic Life Support (BLS/CPR) provider required. Experience in acute care or extended care facility preferred. Able to successfully complete unit-specific core competencies as defined in the orientation check-list. Essential Functions and Responsibilities: ACTIVITY / COMFORT: Assist patients with activity of daily living and provides for patient comfort. This responsibility includes making occupied and unoccupied beds; turning and positioning patients in bed; ambulating and dangling patients; transfers patients; performs passive and assisting with active range-of-motion exercises. HYGIENE: Assist patients with hygiene activities. PROCEDURES: Indwelling urinary catheter care (excluding insertion, removal and irrigation); changing non-sterile dressings; providing ileostomy/colostomy care. Assisting with postmortem care. NUTRITION: Assist patients with nutrition including passing trays, snacks and drinking water; feeding patients; and measuring and recording intake from dietary trays. ELIMINATION: Assist patients with elimination needs recording accurate input and output. Empties other drains as designated by the RN. SPECIMEN COLLECTION: Collects and labels sputum, urine, and stool specimens for laboratory testing; testing urine for sugar, acetone, blood or pH; straining urine for kidney stones; and using the glucometer to test blood glucose levels. VITAL SIGNS / MEASUREMENT: Measures and record accurate vital signs and weight including oral and tympanic temperature, radial and apical pulse, blood pressure, respiratory rate; and using standing, chair, sling and bed scales to measure weight. EQUIPMENT: Uses equipment and supplies safely and appropriately. This includes assisting patients with incentive spirometry; setting up oxygen flow meters; setting up suction equipment and disposing of suction canisters; and applying and removing ice bags, moist or dry heat, ace bandages, arm slings, binders, supports, anti-embolism stockings, sequential compression devices (SCDs), restraints. INFECTION CONTROL: Assists with controlling the spread of infection by using aseptic technique; adhering to universal/standard precautions; caring appropriately for patients with special isolation precautions; passing clean linen and bagging and disposing of soiled linen; replacing full sharps containers; cleaning equipment between patients’ use; and cleaning equipment prior to return to Central Processing Department (CPD). DOCUMENTATION: Document care given as directed by the RN. COMMUNICATION: Communicates effectively using Vocera, unit paging system, telephone system and pneumatic tube system; answering patient call lights; reporting observations to the RN; keeping the RN informed of care delivered; maintaining patient confidentiality. Performs other duties as assigned. These may include but are not limited to: Performing tasks assigned by the Registered Nurse in charge to facilitate department efficiency. Position Summary: The Driver-Certified Nurse Aide is responsible for safely operating the company shuttle, van, or other vehicle in accordance with company policy and transportation laws for assigned communities. This role is responsible to transport residents to authorized locations and provide Certified Nurse Aide care in the event medical care is needed during the transport of residents. Principal Duties and Responsibilities: Driver Responsibilities Provides safe transportation services for assigned communities. Assists all residents to enter and exit vehicle safely. Safely secures residents’ wheelchairs to restraining devices to stabilize wheelchairs during trip. Keeps vehicle clean and free or trash/debris and in good operating condition. May keep records of trips and behavior of passengers. Report resident medical issues in timely manner. Maintain records of weekly inspections of the vehicle. Maintain record of pre-trip, daily, weekly checklists. Attend annual training sessions on defensive driving and van transportations program. Attend annual hands-on-training of proper resident securement and show understanding by presenting a return demonstration Acknowledge by signature the safely responsibilities for authorized drivers. Report accidents and vehicle/property damage immediately to Administrator / Manager Performs other duties as assigned. Certified Nurse Aide Responsibilities: Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality. Provides individualized attention, which encourages each resident’s ability to maintain or attain the highest practical physical, mental, and psychosocial well-being. Maintains the comfort, privacy, and dignity of residents in the delivery of services to them. Interacts with residents in a manner that displays warmth and promotes a caring environment. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the administrator incidents or evidence of resident abuse or violation of residents’ rights. Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items. Observes residents for changes in condition or behavior and promptly reports these changes to the Administrator. Provides care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas. Lifts, moves, and transports residents, using proper body mechanics or lifting devices for accident prevention. Also use wheelchairs, slide boards, walkers, and canes as required. Communicates and interacts effectively and tactfully with the service providers, residents, visitors, families, peers and supervisors. Completes documentation of care provided or other information in PCC in accordance with department policies. Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Attend staff meetings and in-services as requested. Perform other duties as assigned. May be required to support the assigned communities based upon staffing needs when driving assignments are not scheduled.
