Responsible for the planning, delivery, management and evaluation of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.

I. Major Responsibilities:

1.  Assesses and supervises patient care following standard procedure.  Assists in development of patient care standards and resolves discrepancies with physicians and others.
2.  Reviews chart documentation as required or directed.
3.  Establishes recall system for physical examinations, paps, abnormal tests and related results.
4.  Coordinates patient flow and prepares patient for examination.  Prepares patient for office and laboratory procedures as required.  Assists physician, NP, PA and others as required or directed.
5.  Takes vital signs such as weight, blood pressure, height and temperature.  Evaluates patient.  Documents information as required.
6.  Performs and records laboratory results as required.  Documents readings as required.  Ensures laboratory results area received, reviewed and acted upon as required.  Resolves discrepancies with physician and others.
7.  Triages medical calls following prescribed protocols and guidelines.  Documents treatment and presents to physician for review.  Ensures follow-up.  Provides patient education.
8.  Calls prescriptions and refills as required.  Documents prescription calls as required.
9.  Performs med counts and notifies provider of necessary refills in timely manner.  Monitors medications from pharmacy to assure that they match prescriptions and med administration orders correctly.
10. Works well in a team, communicating well and negotiating shared responsibilities with other team members.
11. Demonstrates good communication skills with patients, co-workers, and agencies or providers in the community or who will be involved in the care of our patients.
12. Prepares, administers and documents immunizations, injections and laboratory tests following standard procedure.
13. Coordinates measures for compliance with OSHA regulations as required or directed.
14. Maintains supplies and pharmaceuticals as required.  Orders supplies as required or directed.

II. Position Qualifications:


License/Certification/Education:

Required:
1. Thorough knowledge of the discipline of Nursing.
2. Completion of an accredited school of nursing program.  
3. Current Massachusetts licensure and registration. 
4. BLS is a requirement for every RN unless an advance certification is current (ex ACLS, PALS, NRP).

Job Description: Geographic Coverage: Primarily within Suffolk County, with a focus on Boston neighborhoods (e.g., Dorchester, South Boston, Roslindale, Mattapan, Hyde Park), and coverage in nearby north-of-Boston areas such as Everett, Revere, and Winthrop. The Resource RN provides nursing support to the care team, serving as a bridge between the care management team, hub-based clinical team and members. The Resource RN is available to support and consult on member-specific needs (e.g., in-person visits, clinical education, targeted clinical interventions), and is home based, meaning days will primarily be virtual, but can go into the field on occasion. The Resource RN does not carry a panel, however the RN may work lists of members for targeted clinical reasons. Responsibilities: Support the Member Navigator during the assessment and intake phase to ensure assessments are accurately interpreted and appropriately triaged. Collaborate with care team members for case review and care planning, signing off on care plans, determining escalations, and resolving barriers to effective care. Act as a clinical resource, assisting team members with clinical goals, education, and addressing acute clinical needs. Conduct medication reconciliation, administration, compliance, and education during clinical visits and procedures, including processing medication refills per established protocols or provider orders.
