Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Crown Park Rehabilitation and Nursing Center, a 200-bed Rehabilitation and Nursing Facility located in Cortland, New York. About the Role:

As an LPN at Crown Park Rehabilitation and Nursing Center, you will be responsible for providing direct patient care, administering medications, and performing various treatments under the supervision of our experienced medical team. Responsibilities:

  • Provide direct patient care to residents on the evening shift, including bathing, dressing, and grooming

  • Administer medications and treatments as prescribed by our medical team

  • Perform various medical procedures, such as vital sign checks and wound care

  • Develop and implement individualized care plans with our interdisciplinary team

  • Maintain accurate and detailed records of patient care and medications

  • Collaborate with our rehabilitation team to ensure smooth transitions for patients returning home
