We are looking for a relationship-centered Registered Nurse who is clinically strong, calm under pressure, organized, and passionate about helping caregivers grow and succeed. This role is ideal for an RN who values meaningful connections, collaborative teamwork, flexibility, and the opportunity to help shape the quality of care within a growing home care agency.
This is not a high-volume hospital role or traditional home health case management position. Our RN plays an essential leadership role in supporting caregivers, guiding client care, strengthening systems, and helping our team deliver compassionate, high-quality care every day.
What You’ll Do
Conduct client assessments and reassessments in the home
Develop and update individualized care plans
Provide clinical oversight and support for caregivers
Teach and coach caregivers through hands-on skills training and delegated nursing tasks
Collaborate closely with office staff to support client and caregiver success
Participate in quality assurance, documentation review, and compliance processes
Help improve and strengthen clinical systems, training, and care processes as we grow
Build strong, supportive relationships with clients, families, and caregivers
What We’re Looking For
Current Minnesota RN license
Strong communication and relationship-building skills
Organized, dependable, and able to manage multiple priorities
Comfortable teaching, coaching, and supporting caregivers
Collaborative mindset and willingness to be part of a close-knit leadership team
Ability to handle difficult situations with professionalism and compassion
Experience in home care, hospice, senior care, or community health is helpful, but not required
We welcome strong RNs who are excited to learn the home care side of nursing
Schedule & Flexibility
Part-time: 24–32 hours/week
Flexible daytime schedule
Typically 3–4 days/week
Combination of office and field work based on client needs
