Job Description

Basic Purpose

Positions in this job family are assigned responsibilities for providing direct or indirect professional nursing services in a variety of settings. This includes providing nursing assessments, planning, interventions, evaluations, health related education, case management, and execution of the medical regime including administering medications and treatments prescribed by any person authorized to prescribe by state law.

Typical Functions

The functions performed in this job family will vary by level, unit and organization, but may include the following:

  • Assesses health status of individuals, families, and communities; develops plans and implements appropriate nursing interventions.

  • Evaluates and determines health resources necessary to meet individual, family and community health needs.

  • Delivers professional nursing care in an assigned unit, clinic, home, or other setting.

  • Educates individuals, families, communities, and members of the health care team about the principles of disease prevention and health promotion.

  • Provides supervision to other professional or paraprofessional personnel; collaborates with other professionals in the management of health care.

  • Delegates tasks as may safely be performed by others, consistent with educational preparation and that do not conflict with the provisions of the Oklahoma Nursing Practice Act.

  • Assures quality health care through use of various measures such as record review, peer review, direct observation, and assessment of individual, family, and community for the desired outcome.

  • Performs specialized nursing functions as educationally prepared.

  • Performs other related duties as assigned consistent with the responsibilities of the position and agency operations.

Level Descriptor

Levels are distinguished based on the complexity of assigned duties, the level of expertise required for completion of work assignments, and the responsibility assigned for the supervision of others.

This is the senior level of this job family where employees are assigned more advanced responsibility in an assigned work unit or in an assigned geographic area and may be assigned responsibility for direct supervision. Employees at this level will have complex assignments and a substantial degree of responsibility and independence in planning, organizing, and completing work. Employees at this level develop and present educational programs relevant to nursing practices and standards or care and follow-up with evaluation of learning. This position may include completing special assignments such as quality improvement, mentoring, and surveillance of potential patient care incidents of quality indicators for individuals who may be at risk for rapid status change. Responsibilities may include scheduling work assignments, evaluating health care and performance of assigned staff, providing training, and otherwise insuring quality. Administrative responsibilities may include approving leave and initiating disciplinary actions. Positions which are assigned independent responsibility for coordinating health services or providing nursing care in an assigned county or geographic area or as the only nurse in that location may also be included in this level. Employees at this level deal with complex health issues and may act for nurse managers in their absence.

Education and Experience

Education and Experience requirements at this level consist of possession of a valid Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing and two years of professional nursing experience in a field of nursing appropriate to the health care setting; or Registered Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) and two years of professional nursing experience in a field of nursing appropriate to the health care setting.

Knowledge, Skills, Abilities and Competencies

Knowledge, Skills and Abilities required at this level include knowledge of professional nursing theory, practices, and techniques; of health promotion and disease prevention; of nursing management of available health care resources; of nutrition and diet therapy; of the functions of various clinical departments; of available health programs and resources; of federal and state rules and regulations concerning health services; of accreditation, certification and licensure standards; of preventive health practices; of principles and methods of teaching; of nursing standards; and of nursing policies and procedures.

Ability is required to assist in the development, organization, and implementation of an effective nursing care plan; to provide education and training; to assess condition and needs of patients, families and communities appropriate to the care setting; to take appropriate intervention; to counsel and teach patients and families; to provide direction or guidance to non-licensed personnel in assigned area of responsibility to ensure quality care; to develop, plan, and implement an effective nursing care program; and to provide education and training.

Candidates must possess sufficient proficiency in the English language to perform the essential functions of the position.

Special Requirements

Upon appointment, incumbents designated by the Oklahoma Department of Mental Health and Substance Abuse Services shall complete certification in one of the following required Departmental sponsored training in case management: Certified Behavioral Health Case Manager – Adult; Certified Behavioral Health Case Manager – Child, Adolescent and Family; or Certified Behavioral Health Case Manager – Dual Certification.

Upon a conditional offer of employment, Department of Mental Health and Substance Abuse Services applicants must submit to a physical examination and shall meet the physical examination requirements of occasionally lifting/carrying 50 pounds and able to push and pull a maximum force of 25 pounds.

Applicants must be willing and able to work any assigned shift or day of the week, and additional shifts as required.

Under the direction of the Director of Nursing and in collaboration with the attending provider and interdisciplinary team, the Wound Care RN Lead provides assessment, treatment, monitoring, documentation, education, and clinical oversight for residents with actual or potential skin integrity concerns. The position promotes evidence-based wound prevention and treatment practices, supports regulatory compliance, and works to reduce facility-acquired pressure injuries while optimizing resident outcomes. Typical Functions The functions performed by employees may include, but are not limited to: Perform comprehensive skin and wound assessments upon admission, readmission, identification of a new wound, significant change in condition, and as clinically indicated. Complete required weekly reassessments, documenting wound progress, response to treatment, and recommendations in the electronic medical record. Assess, stage, measure, document, monitor, and manage pressure injuries, diabetic ulcers, venous ulcers, arterial ulcers, skin tears, surgical wounds, traumatic wounds, moisture-associated skin damage, and other complex skin conditions in accordance with current clinical standards. Develop, implement, and evaluate individualized wound treatment plans and resident care plans in collaboration with the attending provider and interdisciplinary team. Perform or oversee wound treatments in accordance with provider orders, manufacturer recommendations, facility policy, and evidence-based practice while maintaining accurate, timely clinical documentation consistent with professional and regulatory standards. Promptly communicate new wounds, significant deterioration, lack of expected healing, signs of infection, and other clinically significant changes to the attending provider, Director of Nursing, and appropriate members of the interdisciplinary team. Monitor implementation of preventive interventions, including repositioning, pressure redistribution surfaces, offloading devices, moisture management, nutrition and hydration support, mobility, continence management, and other skin integrity measures. Collaborate with nursing, dietary, rehabilitation, medical providers, and other interdisciplinary team members to promote wound healing and prevent avoidable skin breakdown. Assess nutritional risk factors that may impact wound healing and collaborate with the Registered Dietitian regarding interventions and recommendations. Provide education to residents and families regarding wound prevention, treatment plans, and factors that promote healing, as appropriate. Serve as the facility's clinical resource for wound prevention and management by providing education, competency validation, and clinical coaching to nursing staff regarding skin 1 assessments, pressure injury prevention, wound treatment techniques, documentation requirements, infection prevention, and evidence-based wound care practices. Participate in wound rounds, Quality of Care meetings, infection prevention activities, and Quality Assurance and Performance Improvement (QAPI) initiatives by monitoring wound related quality indicators, identifying trends, implementing corrective actions, and evaluating outcomes. Administer medications and treatments within the scope of licensure. Perform other duties as assigned consistent with the position and licensure. Demonstrate measurable improvement in staff knowledge, wound prevention practices, treatment techniques, and documentation compliance. 4. Wound Program Oversight and Quality Improvement Conduct routine wound rounds, communicate findings and recommendations with providers and the interdisciplinary team, monitor wound outcomes and trends, and actively participate in Quality Assurance and Performance Improvement (QAPI) activities to improve resident outcomes. 5. Operational Support and Professional Accountability Demonstrate consistent accountability, professionalism, and flexibility by maintaining survey readiness, completing assigned responsibilities within established timeframes, supporting clinical operations as needed, and serving as the facility's clinical resource for wound prevention and management.
