Join Our Team as an RN Clinical Manager!
Are you a dedicated Registered Nurse looking to take your career to the next level? We are seeking an experienced RN Clinical Manager to lead and oversee our home health nursing team. This leadership role involves training, reviewing all clinical documentation, ensuring compliance, and supporting our RN Case Managers to deliver exceptional patient care. The position includes one day of travel per week to visit team members and facilities.
Key Responsibilities:
Training & Development: Provide ongoing training and mentorship to RN Case Managers, ensuring best practices in clinical care and documentation.
Clinical Oversight: Review and approve all clinical documentation, including assessments, care plans, and progress notes, ensuring accuracy, completeness, and regulatory compliance.
Quality Assurance:
Monitor and evaluate clinical performance, identify areas for improvement, and implement strategies to enhance quality of care.
Regulatory Compliance: Ensure all documentation and clinical practices adhere to home health regulations and Medicare/Medicaid standards.
Team Collaboration: Collaborate with interdisciplinary teams to support patient care excellence and promote a positive work environment.
Performance Monitoring: Track KPIs such as EVV compliance, timely assessments, accurate documentation, and patient satisfaction ratings.
Leadership & Support: Act as a resource for RN Case Managers, assisting with complex cases, documentation challenges, and ongoing education.
Qualifications:
Valid RN license.
Minimum 3 years of clinical experience, with home health experience preferred.
Prior leadership or supervisory experience highly desirable.
Strong knowledge of home health regulations and documentation standards.
Excellent communication, teaching, and organizational skills.
Ability to review, analyze, and provide constructive feedback on clinical documentation.
Must have applicable clearances (e.g., background check, child abuse clearances, etc.).
