Qualifications
Current Illinois RN license required
Minimum of 1–2 years of experience in a skilled nursing facility or long-term care setting
Previous supervisory or charge nurse experience required
Key Responsibilities
Supervise and support nursing staff (RNs, LPNs, CNAs) during the PM shift
Ensure residents receive safe, appropriate, and timely care
Monitor and enforce compliance with all regulatory requirements (IDPH, CMS)
Respond to and manage changes in resident condition, incidents, and emergencies
Conduct rounds to assess resident care, staff performance, and unit operations
Provide clinical guidance and support to staff as needed
Communicate effectively with physicians, families, and interdisciplinary team members
Ensure accurate and timely documentation in the medical record
Follow up on pending issues from previous shifts and ensure continuity of care
Participate in staffing coordination and assist with coverage as needed
Address and document employee performance concerns and escalate when appropriate
