Provides comprehensive coordination of care for patients and their families/caregivers. Eliminates barriers to timely care, facilitates flow through system, facilitates post-discharge scheduling, increases patient and provider satisfaction, maintains point of contact with providers and serves as the first point of contact for patients and families.
- Performs assessment of patient care needs for new and ongoing patients, to identify clinical, social and logistical barriers to post-discharge care.
- Oversees the planning, scheduling, implementation and follow-up of follow-up appointments, referrals, diagnostic testing and procedures for navigated patients.
- Central point of contact for all navigated patients; including interaction with medical, nursing and ancillary services.
- Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
- Collects and reports data from patient on barriers, health care, medications, prior treatments, etc.
- Tracks order completion and follow up. Monitors patient compliance with patient treatment plan.
- Assists patients with referrals to outside resources and care coordination as needed.
- Designs or participates in the creation of health education materials.
- Provides and documents quality, personalized and evidence-based care and interventions.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs.
- Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. This includes compliance with all health care regulations, such as HIPAA and OSHA.
- Performs other related duties as required and directed.
- Nursing degree from an accredited educational institution.
- State licensure as a registered nurse.
- Three years of experience directly related to the duties and responsibilities specified.
- Bachelor's degree in nursing.
- One year prior work experience in clinical area.
- BLS certification.
