The Lead Resident Care Supervisor (LPN) supports and ensures the health and well-being of our residents by assisting the DON with overall staffing, staff training and development, and management of personnel in the Nursing department. The incumbent shall assess residents, perform audit measures, and follow state, local and federal regulations and best practices within his or her scope of practice.

Responsible for responding timely to residents’ requests.  Exhibits the highest level of customer service towards residents, staff and visitors.

Consistently demonstrates the ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.

Observes and practices confidentiality of resident information and privacy.

Ensures that residents, staff, and visitors are treated with respect and kindness.

Ensures the rights of residents and personal preferences in care and treatment are respected.

Supervises nursing staff on all med rooms/units/floors while on duty.

Conducts periodic rounds, provides input for the nursing care of residents, acts as a nurse resource person for all nursing staff.

Maintains safe conditions for residents and staff.

Monitors resident care standards during performance of duties and ensures that high standards of care are being provided.

Assists with problem-solving related to staffing, supplies and resident care.

Assists with nursing on-call rotation as needed and assigned.

Appropriately delegates and assigns direct and indirect patient/resident care activities to nursing staff.

Assists DON in ensuring the facility is adequately staffed while maintaining schedules and attendance records.

Appropriately revises assignments and assists with care  based on changes in patient/resident census, acuity and available resources.

Ensures that all nursing staff:

  • completes all assignments
  • promptly reports changes in patient/resident conditions
  • performs timely and accurate required documentation
  • complies with all established policies and procedures of the facility and the nursing department
  • upholds company principles, values and practices

Operated in Compliance with all NYS DOH, as well as state and federal laws and regulations.

Demonstrates knowledge and understanding of all policies and procedures and ability to reference them.

Interprets personnel policies and procedures to nursing staff, upon request.

Observes job performance of nursing staff and provides direct feedback when appropriate; assists DON with staff evaluations.

Performs timely and accurate documentation according to the service excellence standards and goals established for this position.

Ensures and/or assists in performing accurate, thorough, and timely completion of nursing admission evaluations and re-evaluations of patient/resident care needs in accordance with nursing department standards of practice.

Documents as applicable in care plans, nursing notes, medical administration records, treatment records and other required records according to the service excellence standards and goals established for this position.

Provides evaluation of needs and resident care planning information as necessary and requested by other health care disciplines, completes patient education in accordance with established procedures.

Contacts MD/NP/PA for orders or updates according to patient/resident response to treatment plan and/or situation; updates family members as needed.

Participates in Morning Report and Change-of-Shift Report.

Informs necessary nursing personnel of each shift about the current care and needs of residents.

Actively promotes controlled substance and facility safety programs by:

  • Accepts and distributes controlled substances to appropriate med rooms/units/floors.
  • Assists with the destruction of wasted or outdated controlled substances following the bureau of narcotic rules and regulations/as directed by the DON.
  • Performs all position duties according to safety rules and standards of practice required for this position, and as required to ensure the general safety of staff, residents, and visitors of this facility.
  • Actively promotes facility safety programs and serves as mentor/contact person as assigned for staff on leave, etc. to ensure strong communication between the department and the staff members.
  • Performs first aid services and completes Accident/Incident Report of residents, staff and visitors as needed.
  • Implements emergency procedures in a correct and efficient manner.

Reports to DON any unusual symptoms, changes in condition, daily needs, and progress of residents.

Actively participates in the complaint/grievance resolution process by completing requested investigative process and documenting findings in a thorough, timely and concise manner.

Notifies Director of Nursing/Administrator of any significant events/urgent matters related to residents, visitors, staff, or any other disruption to facility services.

Assumes responsibility for nursing services in the absence of the Director of Nursing. 

As directed, takes calls on off hours (i.e. evening and weekend) and adjusts schedule to meet the needs of the facility.

Assists with general staff oversight and management to include interviews, disciplinary actions, education, training and coaching as necessary. 

Executes company policies fairly, consistently, and ethically.

Arrives at work on time, regularly and work as scheduled.

Attends facility meetings, staff meetings and required or preferred in-services.

Participates in Quality Assurance and Performance Improvement (QAPI) programs upon request.

Encourages and participates in the ongoing education and professional development of assigned nursing staff, collaborating with appropriate departments and external organizations regarding education programs.

Ability to participate in facility on-call program which may include working at facility on evenings and weekends.

Utilizes electronic timekeeping system as directed.

Arrives to work on time, regularly, and works as scheduled.

Recognizes and follows the dress code of the facility including wearing name tag at all times.

Follows policy and procedure regarding all electronic devices, computers, tablets, etc.

Qualifications

Lead Resident Care Supervisor (LPN) Educational Requirements and Qualifications

  • Credentials as a Licensed Practical Nurse (LPN) with a current state license in good standing in state which work is performed
  • Minimum of two (2) years of nursing experience as an LPN
  • Minimum of one (1) year of supervisory experience
  • Previous experience in nursing education, training, and/or performing in-services desired
  • Demonstrated proficiency in various computer applications, programs, and electronic medical records
