As the Home Health Aide or CNA, you will provide patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well being.

 

Primary Responsibilities: 

  • May provide or assist clients with activities of daily living including bathing in bed, tub or shower; care of hair including shampoo, combing and brushing; care of teeth and mouth including denture care; nail care, filing only; skin care including peri care and applying lotion; transfer of patient from bed to chair and to wheelchair. Transfer of patients on and off bedpan, commode and toilet
  • Reports observations of the client's condition to the agency director or accounts manager
  • Drives safely, complies with traffic regulations, and wears a seat belt when business travel is required
  • Assists with household tasks directly essential to clients' personal care
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification:

  • Current driver's license, vehicle insurance, and reliable transportation or access to public transportation

     

     

  • NV: Must be certified as a nursing assistant from the state Board of Nursing

     

 

Preferred Qualification: 

  • Current CPR certification or ability to complete within 90 days of hire
  • 6+ months of home care experience
  • Ability to work flexible hours and independently

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements). Is responsible for following the aide care plan, reporting issues related to providing care and patient symptoms as directed in the plan. Education, License & Cert: High School Diploma or GED Required Completion of a NYS DOH approved certification program and is in good standing Experience: Minimum one year of relevant experience. Experience with public contact is preferred. Essential Functions: 1) Updates Knowledge and documentation per agency standards Understands and adheres to GCMC’s compliance standards as they appear in GCMC’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Attends in‐service programs designed to update knowledge and skill related to patient care and to keep abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they change or are modified relevant to job Reads aide care plan at the beginning of each home visit and performs all those tasks listed on care plan and follows policies of LTHHCP in the delivery of care.
