Now Hiring – RN On Call Nurse 

 

Hours: Friday 10a-10p, Sat and Sun 8a-8p

 

The RN-On Call Nurse is responsible for, but not limited to:

  1. Assumes responsibility for a patient visit or on call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process
  2. Provides hospice nursing care based on systematic assessment docusing on pain control and symptom management
  3. Initiates communication with attending physicians, other hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family
  4. Maintains regular communication with direct supervisor to review assignments
  5. Maintains up to date documentation at point of service so that problems, plans, actions and goals are accurately and clearly stated and charges are reflected as they occur in accordance with agency policy and Medicare regulation
  6. Administers medication in accordance with all federal and state laws and the policies of the Hospice along with LTC staff
  7. Maintains current patient records and documents in accordance with the policies of Ascend Hospice to ensure that problems, intervention, goals and outcomes are accurately and clearly states and charges are reflected as they occur
  8. Assess and collaborates with Medical Director to determine initial and on going eligibility of the hospice patient
  9. Completes an initial assessment of patient and family to determine home care needs
  10. Can anticipate end stage disease process and related pain and symptom management
  11. Regularly re-evaluates patient's nursing needs
  12. Uses assessment data to determine care plan needs
  13. Initiates appropriate preventive and palliative nursing procedures
  14. Counsels the patient and family in meeting end of life needs
  15. Performs a comprehensive death note when appropriate
  16. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition
  17. Communicates with other members of the IDT to coordinate the care plan
  18. Participates in On-call duties as defined by the on-call policy
  19. Ensures that arrangements for equipment and other necessary items and services are needed

Job Details Med Services Clinician - LPN (Full Time) MATER This position has a rotating weekday shift, working every third weekend (4 hrs each day) and holidays as needed. Job Description PRIMARY FUNCTION: Under immediate supervision of nurse coordinator in Medication Assisted Out Patient Treatment program for pregnant opioid dependent women, will support medical staff needs by managing medical charts, compliance of clients and all necessary appointments, tracking statistics, general filing, organizing and refining systems of documentation and reporting. Secondary support of medical staff includes medication administration and documentation. Manage required patient medical information, primarily State and Federal requirements for treatment. Acquire documentation of annual requirements from a variety of sources including electronic and paper, schedule for annual requirements. Will establish and update tracking systems, record and track compliance, and issues with compliance and will assist with maintaining compliance, preparing for discharges or transfers. Initiate the medical record by processing the new patient intake procedure: creating the new medical chart, the client ID cards, updating tracking databases, and communicating to other staff any pertinent needs for admission. Enhance medical records and maintain quality results by following a methodical and systematic filing and storing procedure. Maintain the master patient index and all compliance and infectious disease tracking tools by completing assigned portion of daily audit trail; correct and communicate problems according to established procedures. Assist with scheduling all Family Center required medical appointments using IDX, will communicate to the patient via appointments cards and reminder phone calls, will track missed appointments, reschedule and create contracts for completion. Utilize JeffChart, Jeff Signature and IDX/EMR for data retrieval, medication records, appointment verification and scheduling.
